To my wonderful patients:

After 40 years of practicing medicine the time has come to retire. I’m grateful to be able to say I have loved almost every moment of these many years.

We will schedule appointments until June 23rd and will be available for emergencies and administrative issues until June 30, 2022, when the office will close.

If I have seen you in my office since January, 2020, you received a letter via USPS about this in March.

RECORDS TRANSFER

You will be able to obtain your records directly from my office through June 23. This will be electronic records going back to 2012. We will send your entire electronic record to you in an Elation Passport message. If you don’t already have a Passport account, please contact Cindy to sign up. If you transfer your care to a doctor who uses ElationEMR we can send your chart directly to them through Elation.

After June 30

I will not be able to provide your records directly. Your health records will be transferred to Morgan Records Management, LLC (the “Records Custodian”). Requests for records should be directed to Morgan Records Management.

I plan to keep this post office box for a year after the office closes, but I will NOT be able to offer medical care, manage prescriptions, or handle records requests through this address.

This is a bittersweet time. I am honored that you have trusted me to be your doctor and cherish the moments of your lives you have shared with me. We will miss you.

I couldn’t end this letter without honoring Cec, my nurse for 25 years until her retirement, and Cindy, who you all know, office manager extraordinaire, who joined me in July 2016, picking up where Cec left off and enabling me to continue practicing for six more years.
